Q: How is the collaboration between GP and social workers?

A: GP will focus on medical care while social workers deal with case counseling and provide background information.

Q: Any different modes in this Scheme?

A: There are mainly two modes: 1) VMO, GP will conduct an VMO sessions in Neo-Horizon Centre for at least 3 clients each time; 2)GP’s clinic is welcome Neo-Horizon clients for walk-in medical consultation.

Q: How is the referral system for the clients with psychiatric problems or special needs?

A: After medical assessment, GP could write down some clinical observation in their own referral sheet, RN(Psy)of Neo-Horizon will prepare other related referral form with clients’ basic background information as well as some related social description.

Q: How is the payment method?

A: monthly invoice and payment
